CommunityHealth is one of the largest volunteer-based free clinics in the nation that serves the uninsured, underserved, and undocumented in their native languages of English, Spanish, and Polish.

For every $1 donated to CommunityHealth, patients receive $7 worth of free services and medications.

Through our community-based clinics at Enlace and Onward House with plans to open more access points through our new three-year strategic plan, we meet our patients where they are with providers that come from and represent the populations we serve.

CommunityHealth shapes the Future Of Health by training the next generation of health care providers through our Student Run Clinics in partnership with over 15 medical and academic institutions across Chicago.

CommunityHealth more than vaccinates at-risk populations against COVID and the flu but also hosts Question and Answer workshops for patients to learn more about how to protect yourself is protect others.
The most common condition treated at CommunityHealth is diabetes so we offer community-based events that offer free education, classes, and resources like glucose monitors, foot exams. and blood sugar readings.
Screenings for Social Determinants of Health indicate that access to nutrition is limited for communities living without insurance so CommunityHealth partnered with Just Roots to offer patients free healthy cooking classes and fresh produce after a tour of the farm.
Patients contribute to CommunityHealth’s growth, innovation, and strategic plan on the Patient Advisory Council where they share their experiences, ideas, and questions to ensure a quality model of care.
CommunityHealth partners with Elevate to offer patients free hazard screening and renovations in their living situations through the Healthy Homes initiative led by a former patient and graduate of the program.
Students from the Urban Medicine Program (UMED) at longtime partner, ally, and friend University of Illinois at Chicago facilitate a five-week workshop series for patients to learn how to live healthy, fulfilling, and positive lives with diabetes.
The Health Education team offers classes for fitness, nutrition, and community with weekly zumba, yoga, and cooking as well as monthly Women’s Wellness workshops.
CommunityHealth is a revolutionary leader in the free and charitable clinic landscape where we have adapted to, through, and with Medicaid redetermination, the COVID-19 pandemic, and even migrants seeking asylum without shelter.
