We have an especially high need for additional primary care providers at all four of our locations, as well as specialists for the following services at our West Town clinic – gastroenterology, gynecology, ophthalmology, psychiatry, and sleep medicine

Location: Lederman Family Health Center (West Town), CommunityHealth at Enlace (Little Village), CommunityHealth at Onward House (Belmont Cragin), CommunityHealth at The Resurrection Project (Back of the Yards), CommunityHealth at Rincon Family Services (Irving Park), Telehealth (Remote) – options may vary based on services offered
Training: 1hr orientation (onsite or virtual), 1hr self-directed EMR training, 1hr virtual Clinical Orientation, 2-4 hour clinic session with provider champion
Prerequisites: CV, license and certification, Controlled Substance and DEA; if onsite: negative TB test within the last year, Hepatitis B surface antibody titer
Responsibilities: Volunteer physicians, nurse practitioners, and physician assistants are integral to our mission to provide quality primary care and a variety of specialty services to CommunityHealth patients. Volunteer providers are needed in the areas of primary care as well as over 24 specialties.
Schedule/Commitment: At least one 4hr shift/month for one year
