Primary Care or Specialty Care Provider

Our top priority is building our primary care team at CommunityHelath at Enlace.

Providers can see patients from home via telehealth or onsite at 2759 S Harding Ave in Little Village, with shifts available on Wednesday evenings and Saturday mornings. Interested in learning more about our newest location? Email Ava!

We also have high need for dermatology, physical therapy, podiatry, and rheumatology support at our West Town location.

Location: Lederman Family Health Center (West Town), CommunityHealth at Enlace (Little Village),  CommunityHealth at Onward House (Belmont Cragin), Telehealth (Remote) – options may vary based on services offered

Training: 1hr orientation (onsite or virtual), 1hr self-directed EMR training 

Prerequisites: CV, license and certification, Controlled Substance and DEA; if onsite: proof of COVID-19 vaccination + booster when eligible, negative TB test within the last year, Hepatitis B surface antibody titer 

Responsibilities: Volunteer physicians, nurse practitioners, and physician assistants are integral to our mission to provide quality primary care and a variety of specialty services to CommunityHealth patients.  Volunteer providers are needed in the areas of primary care as well as over 24 specialties.  

Schedule/Commitment: At least one 4hr shift/month for one year
