Location: Lederman Family Health Center (West Town) or Telehealth (Remote)
Training: 1hr info session (virtual), 2hr training (onsite or virtual), shadow two class meetings
Prerequisites: If onsite: negative TB test within the last year
Should have general knowledge of diabetes control, maintenance and prevention.
Responsibilities: Diabetes educators facilitate diabetes education classes/small groups for patients and community members using CommunityHealth’s curriculum.
Schedule/Commitment: Classes meet weekly for five weeks for 1.5-2hr sessions. Volunteers commit to facilitating at least two groups of classes per year.
