Your primary care doctor provides evidence-based care, using the latest research to give you the best treatments.

We explain your care clearly so you and your family understand.

Here’s what makes us a patient-centered medical home…

The patient-centered medical home bases your health care on your relationship with your care team.

We will help you learn to manage and organize your care because you are an important part of this team.

For regular care, we help with chronic illnesses like diabetes and high blood pressure.

We also provide preventive care like vaccinations and tests for cancer, yearly checkups, mental health check-ins, and health education to help you manage your health.

For common illnesses or minor injuries and pains, your primary care provider is the best place to start if it’s not an emergency.

You can ask for care/medical advice by calling (773) 395-9900 during these hours:

  • Monday & Wednesday: 8:30am – 9pm
  • Tuesday: 9am – 5:30pm
  • Thursday: 9am – 2:30pm & 4pm – 9pm
  • Friday (September—May): 9am – 4pm
  • Friday (June—August): 9am – 2:30pm
  • Saturday: 8:30am – 1pm

If you have a grievance to share about your experience, we encourage you to report it so we can ensure quality, comprehensive care.

A primary care provider is a general doctor or nurse who takes care of your overall health.

Here are some of the benefits of having a primary care provider…

Relationship: Your primary care provider builds a relationship with you, knows your health history, provides advice, and supports your health decisions.

Connection to Care: Your primary care provider can refer you to other doctors if needed. They can also help you find services like health education, and social support. Check with your care team to know wait times for our referral partners.

There for You: Your primary care provider can often get you an appointment quickly. They can also order medications for you, which you can get on the same day as your appointment in 1-3 hours. If you can’t wait, our pharmacy can hold them for 10 days.

Takes Care of All of You: Your primary care provider learns about your overall health and helps you stay healthy. They test for chronic illnesses like diabetes, cancer, or high blood pressure, and work with you to manage them. They also partner with you to prevent sickness.

We Care about your Care.

Tell us about your medical history and any care you get outside of CommunityHealth.

Keep all your appointments and work with your doctor to meet your health needs. If you need to cancel, call us as soon as possible so we can help another patient.

We promise to treat you and everyone in your community with respect, you should also treat your medical provider, care team, staff, volunteers, and other patients with respect.
