How to Register as a New Patient

We are so happy that you are interested in becoming one of our patients and we are committed to meeting you where you are at with providers you can relate to and services you can understand.

CommunityHealth is one of the largest volunteer-based health clinics in the nation serving the uninsured, underserved, and undocumented for over 30 years.

Low-income adults without insurance receive free primary care and access to over 20 specialties including reproductive and sexual care, medications, lab testing, mental health services, telehealth visits, referrals for resources beyond our medical home, and Health Education workshops, classes, and events.

Please note that CommunityHealth does NOT provide critical care, obstetrics, oncology, nor Substance Use Disorder (SUD) treatment.

Patients in need of treatment for Substance Use Disorder and other complex psychiatric conditions are referred to community partners like Cook County Hospital.

Do you qualify for care at CommunityHealth?

To become a patient at our health care home, you must be or have:

To ensure a smooth first appointment, please make sure to bring all of the following documents to your first appointment:  

  • Proof of income
  • Proof of address
  • Photo ID with current address or piece of mail with name and current address
  • Photo ID
  • License, passport, or Matricula Consular card
  • If the patient has HBIA or HBIS coverage, the HFS approval letter with medical card information
  • 2022 Tax Return or 2022 W2 form

If patient does not file taxes, they may bring: 

    • Check stubs for the past 30 days 
    • OR a written statement from employer if paid in cash 
    • OR supporter’s income documents if supported by someone else 
    • OR bank statements or personal checks from job completed if self-employed 
    • OR unemployment payments for past 30 days

    When you arrive, please make sure to stop by the front desk to check in for your appointment. Whether you’re here for registration, specialty exams, nurse visits, or health education, it’s important to complete the check-in process at the front desk. 

    When filling out the registration form, please be sure to include your email address. This detail is essential for setting up your Patient Portal, offering you easy access to diagnostic and test results, and facilitating more efficient communication with your care team. 

    For those fluent in Spanish or Polish, our clinic offers interpreter services. However, if English, Polish, or Spanish is not your preferred language, we kindly request that you arrange for your own interpreter to accompany you during any visit to our facility.  

    Eligible patients are welcome to use our onsite pharmacy for free. If you don’t meet the eligibility criteria or need specific medications that we currently don’t have, you can choose to use other retail pharmacies at your own cost.

    Please remember that while we can help with outside prescriptions, we can’t fill scripts from other places. We can provide a 90-day supply of medications tailored to your medical history, but this may vary from patient to patient.

    You can choose to pick up your medications on the day of your appointment; just make sure to let the front desk know about your plans. Same-day pick-up hours can range from 30 minutes to few hours. If you prefer to come back on a different day for medication pick-up, feel free to do so.

    Just remember, you have a 10-day window to collect your prescribed medications from the date the doctor orders them. Be mindful that pharmacy hours are different from clinic hours, so always check our website for the right medication pick-up hours. Stay informed for a smooth and convenient experience. 

    All of our services, including TB tests, physicals, and one-time appointments, are exclusively accessible for registered patients. We do not provide services for individuals who have not completed the registration process with us.
