In one week, CommunityHealth will officially have three locations! Enlace Chicago, located in Little Village, is our newest point of access for patients to receive quality care.
Just like our co-location at Onward Neighborhood House in Belmont Cragin, CommunityHealth at Enlace will be a home to state-of-the-art technology, with which patients can connect to their medical providers via telehealth. There will also be a medical assistant on-site to take vitals, draw labs, and assist with technical difficulties.
Our space at Enlace includes a large atrium area, as well as two private rooms that will be used for office and medical exam spaces. Patients will be able to access both telehealth and in-person medical care at CommunityHealth at Enlace. The following photos are from our recent New Patient Registration Fair, which took place on Saturday, May 21st.
For our pre-existing patients who live in Little Village, Pilsen, and Lawndale, this new location is much closer to home and much more convenient. For other community members who are already familiar with Enlace, the arrival of CommunityHealth is an exciting new opportunity to receive health care in a trusted location. Thanks to Enlace’s Community Health Worker program, they have established trust among residents of Little Village. We hope to grow our patient network and bring health care to the population that Enlace has been serving since the 1990s.
Opening the doors to CommunityHealth at Enlace also has exciting impact on our volunteers: the opportunities for telehealth volunteerism abound! Now providers can enjoy the flexibility of providing care from wherever they are. Furthermore, based on what we’ve seen over the last two years, the “no-show” rate for telehealth appointments is significantly lower than for in-person appointments. Not only does this indicate the convenience of telehealth for our patients, but it also means that providers are less likely to come in for a volunteer shift and end up with a last-minute cancellation.

Neither CommunityHealth at Enlace nor CommunityHealth at Onward House would exist without the support of The Health First Collaborative (HFC). In an effort to combat “the twin pandemics” of COVID-19 and racism, the HFC has created specific action plans with participating safety net clinics here in Chicago, including (but not limited to) Chicago Family Health Center, Family Christian Health Center, Esperanza Health Center, and of course CommunityHealth. HFC’s sponsorship of our co-locations has been monumental, and we want to extend a warm thank-you to them as we prepare to open our doors to the Little Village community.
